Rates and Insurance

Intake appointment (first appointment) - $200.00

Individual and / or family therapy, 30 minute session - $100.00

Individual and / or family therapy, 45 minute session - $135.00

Individual and / or family therapy, 60 minute session - $180.00

Interactive Add-On - $50.00

Psychotherapy for crisis (each additional 25-30 minutes) - $70.00

Excessively failed appointment (see informed consent – per instance) - $75.00

Neuropsychological testing (per hour) - $180.00

Psychological testing (per hour) - $180.00

On site consultation performed on behalf of a patient (per hour) - $160.00

Consultation provided by telephone (per hour) - $100.00

Preparation of treatment summaries, letters, other paperwork (per hour) - $100.00

Retainer for court services (Determined on individual basis)

Court preparation and testimony (per hour) - $200.00

Copies of clinical records (per page) - $0.25

Travel Time (round trip/per hour) - $200.00

*** Please note that your fee may differ slightly from those listed above for therapy if you are a client insured by one of the insurance companies with whom Dr. McCaffrey contracts and you ask that we bill your insurance for services rendered.

Cancellation Policy (see informed consent)

Excessive appointment failures (no shows or late cancellations) may result in a charge for the missed appointment. Dr. McCaffrey will discuss this issue with you if no shows or late cancellations are approaching the point when a charge will be imposed.


Insurance

Dr. McCaffrey is a provider for most major insurance companies. If you do not see your insurance company listed below, please email or call Dr. McCaffrey's office to verify. You may also check the approved provider list on the insurance company's website to confirm whether Dr. McCaffrey is a provider.

Dr. McCaffrey is currently a provider for the following insurance companies

  • Aetna
  • Blue Cross Blue Shield (BCBS) PPO policies only
  • Cigna
  • Community Care PPO or Preferred Community Choice
  • Healthchoice
  • FirstHealth (formerly PPO Oklahoma)
  • Medicare
  • Medicaid or Soonercare
  • Tricare East Region only
  • United Behavioral Health / United Healthcare

If you have one of the insurance plans listed above, your company will probably reimburse Dr. McCaffrey at a higher rate than if you went to a "non-provider." If Dr. McCaffrey is not a provider for your insurance company, billing staff can usually still file claims with them and will check your out-of-network insurance benefits for you. Exceptions to this are if your insurance plan is an HMO. Dr. McCaffrey is not a provider for any HMO's and, typically HMO's do not reimburse for services by out-of-network providers. As such, you would need to pay for the full cost of services yourself.

Health insurance represents a contract between the insurance company and the family or individual. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by calling your insurance company and asking the following questions:

  • Do I have mental health insurance benefits?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • What is the amount of my co-pay and / or co-insurance?
  • Is approval required from my primary care physician?
  • Is prior authorization for services required?

It is the responsibility of the client and his / her family to know the limits of their coverage. Failure to learn these limits does not relieve the client / family from their financial responsibility. As a courtesy, administrative staff will request benefits information from your insurance company. However, Dr. McCaffrey is not financially responsible for any incorrect information given to staff. You are responsible for any fees or portion of fees not covered by your contract with your insurance company. (Determination of benefits by this office is not guaranteed.)

You should be aware that your contract with your health insurance company likely requires that Dr. McCaffrey provide it with information about his services to you. He is usually required to provide a clinical diagnosis and dates of treatment. Sometimes he is required to provide additional clinical information such as treatment plans and / or summaries or copies of your entire Clinical Record. In such situations, Dr. McCaffrey will make every effort to release only the minimum information about you that is necessary for the purpose requested.
